Fee-for-Service

Patients have the freedom to use the doctor of their choice. These plans generally will pay a percentage of the charges to the provider after the patient has met their deductible. The patient is responsible to pay the percentage that the insurance company does not pay.

HMO (Health Maintenance Organization)

An HMO is a prepaid health plan. You must select a doctor designated by your HMO from a list of doctors in that network (in-network provider). Depending on your plan, you may pay a co-payment and other out-of-pocket expenses.

PPO (Preferred Provider Organization)

A PPO is a combination of fee-for-service and an HMO. You may select a doctor innetwork (one designated by your insurance) or out-of-network (a doctor of your choice). Generally, your out-of-pocket costs are likely to be somewhat higher if you chose an outof- network provider.
